New Incisionless Weight Loss Procedure - POSE, now available in Southampton

POSE (Primary Obesity Surgery, Endoscopic) is the first incisionless surgery for obesity to be offered in the UK and is now available privately in Southampton. With this new procedure available to patients with a BMI of 30-40 this may widen the availability of bariatric surgery options to those patients who do not meet international bariatric surgery eligibility criteria and who have struggled with controlling their weight through diet and exercise.

Bariatric surgeons, Jamie Kelly and James Byrne have been trained in Houston by GI Dynamics, the device manufacturers in POSE, and are poised to become the first team in Europe to offer this new incisionless weight loss procedure to decrease the capacity of a patient's stomach.

This is the first procedure of its kind to be done without having to cut into the abdomen to reach the stomach. Incisionless surgery may offer a number of benefits including less pain, a shorter hospital stay, a faster return to normal activities and no scarring.

POSE is performed under a general anaesthetic, by endoscopy. The procedure itself involves placing tissue anchors in the upper part of the stomach which have the effect of reducing the capacity of the stomach so that patients feel less hungry after the procedure and will feel full after eating a smaller amount of food, and hence be helped to lose weight.

Avoiding incisions in the skin means that the procedure is associated with minimal discomfort, rapid recovery and minimal if any time off work,

As this is a new procedure, patients will have procedures performed in the operating theatre and be kept in hospital overnight. It is anticipated that once the team has gained experience of this procedure it will be performed as a day case in the endoscopy unit.

POSE has now been performed in approximately 70 patients to date in the US, and has proven to be safe, with no serious complications.

Early results (up to 12 months) suggest that POSE is effective in promoting weight loss in the overweight who have a body mass index of 30-40 (Mean excess weight loss 36% at 3-12 months).

POSE is aimed at overweight patients for whom the more radical conventional approaches of gastric banding, bypass or sleeve gastrectomy may not be suitable or acceptable.

It is very important that anyone considering this procedure is aware that whilst the early results in terms of weight loss are encouraging, that there is not yet any medium or longer term outcome data. As with any weight loss intervention, in order to get the best results patients need to work with the bariatric team to change eating and physical behaviour to achieve the best result.

The team will be carefully recording the results for all patients and will present and publish this information in due course to help define further the short medium and long term effectiveness of this procedure.
