Tier 3 Weight Loss Management Programme

Hello All

New here and wondering if anyone has been referred for tier 3 weight loss programme? I have a bmi of 48 but considered quite healthy as no other problems! I am 40 years old and been over weight since the age of 10! I have constantly approached the doctor year upon year for further help I have succeeded once losing a total of 8 stone and still weighed in at 14 stone but never managed to keep off! I am now back to my heaviest and just find it so hard to keep weight off! I believe I am addicted to food and really crave nice food - I dont consider myself to be a huge binge eater just make the wrong choices! My doctor last year told me I should perhaps just be happy the way I am but after having losing all that weight once and being the skinniest I have ever been since I can ever remember it has just made me want to lose weight even more and I can notice I am unable to do things now because of my size! Truly depressing!!!!!!!!!!!!!! I sought another doctors advice and was referred to this tier 3 programme but after an initial phone call a bit disheartened as it is basically going over all that I already know and not a long term solution! I need surgery and if I could afford myself I would be booked in tomorrow! Does anyone know if this could lead to surgery???? I am in Hampshire!:cry:

Hello, yes unfortunately that's what they seem to do now. I was referred for what I thought was surgery early oct but after a consultation late nov I was told that I have to do a 12 week weight management course first, lose 5% then when I go back to him he will put in for funding and start the process. I have my first appointment for this in March. Areas seem to vary with some being much quicker and others a bit longer. Good luck for a speedy journey! :)

Hi Lynnepoth,

I'm in Southampton but have done everything up in Leicestershire. The Tier 3 programme is the first step in many journeys to eventual surgery. I was first referred by my GP in April 2013. In May I had my first appt with the dietitian, who said I had to lose at least 5% of my body weight before referral to the surgeon. After 6 weeks I had lost almost 5%, but I had to go to Australia for 10 weeks for part of my uni course. I saw the dietician on Halloween, having lost nearly 10% and therefore completed the Tier 3 programme, he referred me to the surgeon who I saw the week before Christmas. I had surgery on the 7th Feb.

The whole point of the T3 programme is that you can show you're willing to change your eating habits long term, and can stick to a diet that is more 6x small meals a day rather than 3 normal meals a day - that's what's required after surgery, regardless of which type of surgery you have :)

Thank you both for your replies! I just felt a little disheartened but hoping that I on the right route. Do I need to ask about surgery at my appointment? I just feel that I need something more and I have read lots online about surgery and know that it means a complete life change I just need that extra extreme action to keep me focused! I have lost 8 stone before and kept off for about 1.5 years but lost track again and could not get any lower than 14 stone! I think there is more to it with me as always been big right from a toddler and do honestly think that I don't always over eat my body just does not burn it as much as others!

Hi Lynnepoth, im in portsmouth and also had to complete the tier 3 programme for 12 weeks. I waited 8 months for a space on the programme and completed it 3 weeks ago, losing 4 stone. At the end of the programme I saw the tier 3 doctor who wrote to my gp recommending referral. My gp did the referral on 28th Jan and I got an appt thru today to see the consultant on 4th April. In portsmouth once the tier 3 is completed you also have to attend 3 surgery prep groups which are held monthly...lots of hoops to jump thru but hopefully get there in the end...good luck x

You only done 12 weeks I Have been told 6 months for this course and started last week! Can I ask you all what bmi you all were when started? I just hope that this will lead to surgery

Hi, mine starts this week. I was told 12 but I know some say 6 months. I think it might depend if you have co morbidities such as high blood pressure diabetes etc. My bmi is currently just under 49 but was 54 at referral . Good luck with the course! :)

Did you all ask about surgery on your first appointment I asked if it leads to surgery and she just said it can do ! I just feel as if this is not gonna do anything! I know all this how and what to eat I either lose all will power or can't substain! If had the opportunity if surgery to help weight loss now I know I would never go back at the moment j am sick of people saying j don't look my weight and I am otherwise healthy so why am I bothered! I am bothered cause I will end up ill or dead!

my bmi was 60 when i started weight management, but other than my weight I am healthy, no other issues at all. I had to do 3 months at weight management then attend 3 monthly sessions of surgery preparation, ive done 1, got another this week and then one in April. I made it clear I wanted surgery from the start but you still need to prove you are willing to make changes to get the referral.

I to am in portsmouth, I did the life morph/bodymorph programme for 24weeks/ 6months is that considered the start of the tier 3 weight management programme, i have a surportive gp who refered me to the bariatric physician for further advice, i was going to try and have chat with the bariatric surgeons at work( i work in a hospital) but have not been able to for one reason or another,

I will keep on trying to get approved for surgery and would say good luck in your journey.

Does anyone know what the tier 3 programme actually involves

I have just started tier 3 but not sure whether this is path to surgery and doctor did not know what it involved! I have an allotment with dietician one with physcologist and one with fitness lady! To be honest nothing I don't already know!!!!!

Body / lifemorph is the name for the tier 3 weight management in Portsmouth, so your on the programme. Mention to them that you are hoping for surgery so that they know from the start. You initially do 12 weeks where you attend approx fortnightly to see exercise lady, psychologist and dietitian in turn, then if you have followed the programme and lost at least 5% of excess weight they then write to your GP asking them to refer you to the surgeon team. After the initial 12 weeks you then have to do another 12 weeks where you attend monthly surgery preparation groups, but you can do these whilst awaiting an appt at the hospital. Its so complicated!!

thanks for that info,

i know talking to patients who come in for surgery at work they say its been a long time of preperation to get to the surgery point.

when i did the life morph programme i would see some one at least every two weeks mainly the exercise lady who does all the measurements when you forst start, then the dietitian and saw the pshycologist person maybe two or three times but mainly saw the other two ladies.

have my first appointment in just over a week or so and so can't wait to get started on the road to surgery

starting to get nervous

Hi I have the appointment with the bariatric physician next tuesday and am beginning to panic,

I am afraid that she will say i have to do the whole body morph thing again even though I did it for six months I have been going to sw for more years than i can remember and and have lost and gained the same 1.5stone and am thinking surgery is the only thing left that will help me.

I am thinking of taking my weight record from slim world with me so she can see whats going on.

I have already cut my portion sizes and cook from scratch and use a low fat cooking spray I really don't know what to do now.

I have done the weigh ahead, 3tier weight manger net programme, finished it in march,lost 24lbs, my doctor referred me to this, the end result was I could mange by myself , I was soo upset, so went back to my doctor who referred me back to the hospital, I'm now being funded for surgery,met my surgeon today, date has been set. Mixed feelings,

I'm a newbie and just saw your thread whilst scanning through the forum. I'm Eastleigh area and have just completed my paperwork from GP to be referred for tier 3 "the weigh ahead" programme which appears to be sponsored by Spire Southampton. My BMI is 46 and like you have yoyo'd with weight forever!! Finally decided I need radical help and am pleased that my GP has supported me in this first step.
Just wondered if you are Spire Southampton and are now on the tier 3 programme? If so, can you share any infor about it- is it 6months as stated in the paperwork?? Have they mentioned about surgery or waiting times for anything?? How long has it taken to get onto programme?
Sorry so many questions, but just so many questions and so many emotions!!

Hi! Nice to hear from someone nearby! Yes spire it's with! To be honest seen dietician 3 times now they don't tell me anything I don't already know! I been going since April not lost any weight think I am due to finish September! You get 8 gym sessions not sure how that will achieve anything! I asked about surgery and yes it can lead to surgery so time will tell! Always been big myself and only once managed to lose decent amount if weight doing lighter life but due to health issues gained all the weight do back to square one!

Hi Lynne, I've also had to do the tier 3 thing at Spire, so if you have any questions, let me know! I live in Southampton now but I was in Poole when the whole thing started. Everything that the dietician said I also have heard but of course, didn't let it show lol

I did it for 6 months, saw the exercise guy once, therapist 5 times and dietician 6 times. Only lost a few pounds and was recommended for surgery. I think they do take your mental state into great consideration, they basically want to see you're at least trying. Hope it all goes well for you!

Can't wait to get first appointment now just to find out what it involves and just get ball rolling! Do you think I should ask about surgery when I go for first session and say I'm interested or is that not a good thing to go into it asking about?? I have been in contact with the tier 3 weight management administrator and she said the first appointment will be 8.30-3.30!?! I thought it would be max a one hour appointment!! Xx
