PCT funding application, please help me plan

Hi,

I have an appointment with my GP to plan my application for my PCT funding. I have already been recommended for surgery by the consultant,

Can anyone help with ideas. I dont want to miss anything and have to appeal.

Has anyone got a copy of their application that they would like to share with me. I would appreicate any help.

I spoke with the obesity nurse today and she said that I have a very good case for my surgery and that I shouldn't worry but I can't help it. It has been such a battle to get this far!
 
Hi there, I went privately for my bypass as my pct refused on the basis that I was not morbidly obese enough:confused: I know a few ladies on here have fought and won for funding and Im sure they will pop along shortly to give you the low down on what to put and give you some really good info.
Best of luck, if the surgeon and gp are on your side then thats half the battle won!
 
My GP wrote to the PCT for funding, there is no form to fill in as far as I know. He writes to them stating why he thinks you should have the surgery and what you have tried so far to lose weight. Hope this helps...Linda
 
Ditto, except I went through the local bariatric co-ordinator and she did it all instead of my GP, but thats just how my PCT works.
 
I know a few people on here have written letters and asked the GP to include them with the application. Basically saying why they want the surgery, problems obesity causes them, how much money it would save the PCT etc. Maybe you could try that?
 
If they require more information they contact your GP and he supplies it. They can say no the first time around but there is a process to appeal. Not saying that that is what will happen, they did not say no to me they said they wanted me to attend weight management clinic for two years. My GP has written back to see if they will take into account that I have been trying with him and other methods, ie weight watchers, slimming world, and with a weight management clinic already. We are still waiting to hear back. I have seen a consultant and if when I see the surgeon he also recommends surgery they will write to my GP and on that basis he will contact the PCT again. The best way forward is not to give up without exhausting all avenues, I bleat and moan but will fight until I get what I want....if your GP supports you thats a start...Linda
 
The GP gave me the form and I completed it, returned it to him and he sent it off.

I kept a copy but don't have it with me.
 
My Gp sat with me and agreed what was going to go into my application. That included my depression, low self esteem and high BP and diabetes. Alongside this he evidenced my previous attempts at weight loss and failure for me to maintain any weight loss. Just make sure you have a list of things you feel would improve should you be successful in obtaining the funding.

Hi Gingercookie, My GP told me that he applies for funding, so there might not be an application form to fill out. Hope everything goes OK. I also have a question for you, after seeing your GP for the first time how long did you wait for an appointment to see the consultant who gave you the ok for surgery?
 
I wasn't referred to the surgeon until after the funding was in place. At that point, there was a strict timescale set by the PCT so that I had to receive an appointment letter within 10 weeks, and had to be seen within 4 months. The problem is everywhere is SO different! Things happen very quickly in some areas, and very slowly in others (took me over 18months from first seeing my GP)
 
My GP refered me to WMC who recommended me to have WLS they then wrote to my GP asking him to secure funding. I went to see him before that letter aeeived and toldhim what was happening and he checked that it was what i wanted. Once the letter arrived from the consultant he sent a copy of the letter to the PCT asking them for funding.
Has yr specialist given you a letter outlining why he thinks yr suitable?

I have been attending my doctor for years regarding my wieght battle. I asked him 18 months ago could I be referred to the hospital to the obesity clinic at Luton. He wrote to them and they turned me down. They insisted that I attend wieght loss appointments with the nurse. I did this but it didn;t help. They also wanted me to try the wieght loss medication. I did try Xenical years ago, I lost almost 7 stone but I have regained every pound and more!!! I have also tried reductil and acomplicia, both of them made me very depressed and made me unwell. I wrote to my doctor explaining how down I felt and I begged him to refer me again. This time they said yes they would see me. I had to wait 3 months for that appointment. I rang them for a cancellation which arrived the next week. From there I seen the obesity nurse. After 3 visits with her she referred me to the consultant. He did not hesitate in recommending me for the surgery.

Tomorrow I will talk to my doctor about my application for funding. I hope it goes well. Can any one offer me any help. I would appricate any advise.
 
re funding

hi just been reading your thread.i first discussed.with my gp this time last year and was refered to a endocrinologist who i saw in november 07 i then had many blood tests sleep tests etc.the consultant would not see me until funding was in place.this was agreed.in july08 and i now have an appointment next week.at no time was i asked to complete any application forms etc
 
what is a endocrinologist ?

I haven't been asked to apply myself. My doctor is going to apply on my behalf.

Glad to hear you are so near getting your surgery.
 
it is a consultant attatched to the weight loss clinics their main role is the care of diabetes, obesity and other chronic diseases necessitates understanding the patient at the personal and social level as well as the molecular, and the physician–patient relationship can be an important therapeutic process. (Not my words wikapeidias)
 
I am still waiting for the consultants letter to arrive at my doctor so my GP can get on with my funding application. I have decided to write a letter to my doctor and ask him to send it with the application. I believe my BMI is 49 today.

Someone has told me if my BMI is 50 then I wouldn't need funding. I rang the obesity nurse and she wouldn't confirm this. How can I find this infomation? I live in Luton.

I also need to find out more about the PCT funding in my area can anyone advise me on where to look?
