Questionnaires to complete

Well I got home tonight and found an envelope with a Chichester postmark. Eek I thought! Inside were three questionnaires which, from what I've read, will give an insight into how you feel about yourself, your self-confidence and your history with food. The letter says to return them within 2 weeks otherwise they will not progress further with your treatment. It also says that on receipt of the questionnaires and discussion by the multi-disciplinary team, one of 3 things will happen:
1. A one-hour consultation with the specialist psychotherapist at the hospital.
2. If they have any questions they may contact your GP for further information or phone you to arrange a short telephone consultation with the psychotherapist.
3. The psychotherapist will deem you suitable for surgery and recommend this to your surgeon.
So, at last, things are happening. Hopefully, if I answer the questions positively, option 3 will be chosen and its all systems go!!! Although whether I would still have to see a dietician or not, I don't know.
Hope this bit of information is helpful to others like me who are waiting for St Richards and feel like nothing is happening.

I had one of them forms to fill as well but it took me 3 weeks to fill because it was that long and I was feed up doing it . I seen their psychotherapist and I was only in with her for about 10 mins . She just asked the questions that was on the form and about positive things I want to do after the surgery , I cant swim and Id like to learn and Id like to start horse ridding after the surgery . Well I completed mine last night and sent them off today. Out of interest, anyone else who got them from Chichester - did you notice the typographical errors in the American test? Now, whether this was a test to see whether anyone actually spotted them or it was just a silly mistake, but I had to correct the mistakes before completing the questionnaire!!!! I answered all 3 questionnaires honestly (thank goodness I've been feeling pretty good the last few weeks otherwise my answers may have been different!) so hopefully that will go in my favour and I'll get straight to the op without seeing the psychotherapists. Although, what happens if I don't see the psychotherapist? Is it straight to the op, or is there a pre-op appointment first where they do bloods, swabs etc? Julie x

ps: what sort of questions are on this questionnaire?
 
Hi snowcrystal - I can't remember the exact questions but its sort of multiple choice - eg- how do you feel about youself - a/ I feel fine b/ I feel ok c/ I want to kill someone d/ go ahead punk make my day - that sort of thing. You score 0 points for answer a, 1 point answer b etc etc. I think the idea is that the lower you score the better. That sort of applied to 2 of the questionnaires, the 3rd was background into your weight problems, childhood history, that sort of thing. Must admit I completed it and sent it off without taking a copy so I can't remember what I said now!
