for what ever reason
if so
what was the cost approx if was funded by your self

Having it removed costs about the same as having it fitted, unfortunately. It certainly does at THG anyway:

Out of curiosity, if anything goes wrong with the band are the nhs obliged to remove it?

I'm just going through the same process I had my band done by spire privately 5 years ago and since then I've acquired a hiatus hernia and now nhs have suggested taking it out seeing the bariactic team in oct!! X
