tier system

The NHS lists the different tiers as follows:

 Tier 4 - Specialised Complex Obesity Services (including bariatric surgery)
 Tier 3 - A primary/community care based multi-disciplinary team (MDT) to
provide an intensive level of input to patients.
 Tier 2 - Primary Care with Community Interventions
 Tier 1 - Primary Care and Community Advice.

I think most people when they approach their GP have done Tier 1 and 2 i.e. seen the GP at some point in the past about their weight, have previously tried diets including formal groups such as WW or Slimming World. My experience was that when I saw the GP to ask about weight loss surgery he could see by my history that I'd always been obese, had sought advice on several occasions and had attended formal weight loss groups so he referred me for Tier 3.
 
Iv done every diet including a few through the gp so he's referred me but nothing was mentioned about tiers. I presume I'll be tier 3 too.
 
I saw G.P and he sent me to local hospital to see dietician and i also had 6 sessions with the community Health trainer at the health centre.
Once i told the Dietician at my localhospital that i was waiting for my first appointment at derby for WLS she wrote to my G.Pand said she thought it better i i only saw the team at Derby, and discontinued my visits with her.She did say however if i needed her for any advice post op or inbetween appts she was there for me.
The sessions with the Health trainer were more about Healthy options, salt etc than actual weight loss.I started the 3 tier this month and my next appt is on 29th Oct for personal assessment .I will be under Prof.R.Donnellys team.Anyone know of him? (Derby).
