Advice needed

Hi guys
Just some advise really

I followed and completed the tier 3 programme in my home town and have been referred to Luton for tier 4 surgery with a bmi of 58 and 2 of the chronic conditions... I have an appointment in April 2017 just wondering what to expect and timeliness as like I said already completed the Milkids diet

Any helps and tips great fully recieved x

Massively different .

I don't understand the tier system as I never had to do it with my NHS provider .. xx
